Greetings
 
I have tried my level best to secure a copy of the annual food inspection report that our town is required to submit to the department of public health. From my previous enquiry with MA DPH, I infer that there were gaps in the filing. Mr. Herbert had informed me (see 14 May 2017 email listed below) that the missing years are 2014 and 2015 further he states that Mr. Mark Oram has completed them. I would appreciate if you can kindly help me secure the electronic copies of the annual report submitted in 2017 for the year/ 2016 , 2015 and 2014. Please treat this email request as a public records request and please do let me know the cost for processing this, I do not want the town to spend funds for this even though my request is aimed at doing my task as a board member. I would like to compensate the town for the time spent on this request.
